Cityworx Limited, in partnership with a leading Home Care Provider, is seeking a dedicated Community Lead Nurse to oversee a number of clients in Hertfordshire and surrounding areas. Our client has a long-standing relationship with the NHS, providing care services to individuals with continuing healthcare needs who can live at home among their family and friends.

We are recruiting a Community Lead Nurse or a suitably qualified medical professional, such as a Nurse, Paramedic, or other qualified individual, to train, advise, maintain staff competency, and have overall clinical oversight for client contracts. This role offers a unique opportunity to lead a team of passionate healthcare assistants dedicated to providing high-quality community care. The successful candidate will work alongside the Registered Manager and senior management team, who are themselves clinicians, to monitor the service and develop processes and procedures.

Full-Time, Hybrid (part office-based, part field-based) 

Key Responsibilities:

Leadership and Management:

Lead, support, and inspire a team of community healthcare assistants to deliver exceptional patient care. Oversee the coordination and management of nurse-led services within the community, ensuring seamless care delivery. Develop and implement nursing protocols and standards in line with best practices and regulatory requirements.

Patient Care: 

Ensure nursing care is delivered to patients in their homes with a person-centered and empathetic approach. Perform thorough patient assessments, create individualized care plans, and monitor patient progress. Collaborate with multidisciplinary teams to ensure comprehensive and cohesive care.

Quality Improvement:

Continuously evaluate nursing practices, fostering a culture of evidence-based care and innovation. Monitor and report on key performance indicators, ensuring adherence to quality standards. Engage in quality assurance activities and audits, identifying improvement areas and implementing solutions.

Community Engagement: 

Establish and maintain strong relationships with patients, families, and community partners. Advocate for patient needs and ensure access to necessary resources and support. Participate in community health promotion initiatives and educational programs.

Business Development: 

Contribute to the development of a complex care and live-in care division. Oversee both complex care and live-in care areas and conduct field visits to ensure quality and compliance.

Qualifications:

Education: 

Bachelor's degree in Nursing (BSN) or equivalent; advanced degrees are a plus.

Experience: 

Minimum of 2 years of nursing experience. Current NMC PIN. DBS clearance.

Train the Trainer qualification is desirable.

Prior experience in community health, home care, or similar settings is advantageous.
