We are looking to recruit a Qualified Social Worker in the Community Mental Health Team within a London Borough of Tower Hamlets. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels.

The responsibilities of the Social Worker includes:

  • Working in integrated/partnership within the Community Mental Health Team/East London NHS Trust
  • Provision of a social work service within the framework of the Care Programme Approach & Care Act plus other appropriate legislative requirements
  • Working with individual service users in hospital and in the community including placements providing advice, assistance, assessment, review, safe guarding enquiries or other forms of social work with the aim of enhancing the outcomes for service users and carers
  • Develop support plans which meet personalised outcomes
  • Assist individuals, carers and families by completing strength based assessments which support the identification of individual, personal network, community strengths and universal services
  • Attend case conferences, case meetings and team meetings as required.

To ideal candidate will have:

  • Social Work England registration
  • Eligibility to work in the UK
  • Social Work qualified (MA/BSc/DipSW/CQSW/MSc/BA)
