About the Role:

P4 Recruitment are seeking a dedicated and compassionate individual to join our client's team, working collaboratively to support individuals on their recovery journey from drug and alcohol misuse. This role involves providing comprehensive recovery planning and case management across all stages of recovery, from initial assessment and engagement through to sustained recovery.

Key Responsibilities:

  • Manage a caseload of service users, developing, implementing, and regularly reviewing recovery plans and treatment strategies. Liaise closely with a diverse range of team members, including family members and significant others when appropriate.
  • Conduct triage/screening assessments and comprehensive evaluations with service users.
  • Collaborate with partner agencies to perform shared assessments, key working, and interventions aimed at promoting individual progress from initial engagement through structured treatment to self-sustained recovery.
  • Facilitate service user access to treatment and community resources, enhancing recovery and social capital by supporting service users and their families within their local community.
  • Lead effective case management for allocated service users, which includes conducting ongoing risk assessments, developing risk management plans, and safeguarding assessments. Utilize an online case management system efficiently.
  • Work closely within a multi-disciplinary team, both internally and externally, providing reports and information to managers and colleagues through reviews, team meetings, supervision sessions, and telephone conversations.

Skills and Qualifications:

  • Strong verbal and non-verbal communication skills, with the ability to engage effectively with clients about their drug use while maintaining professional boundaries.
  • Ability to establish and maintain positive and productive working relationships within the team and with external partners.
  • Proficiency in MS Office programs, including Word and Outlook.
  • Experience with case management systems (desirable but not essential).

Knowledge and Experience:

  • A thorough understanding of the complex needs faced by individuals with a history of problematic alcohol or drug misuse, including physical, psychological, mental health, legal, parenting, and family support needs.
