Are you a dedicated RGN Qualified Nurse with a passion for complex care? Our client is seeking a Community Nurse Case Manager to support clients in Lincoln and the surrounding areas. This role offers a unique blend of home-based work and community visits, ensuring clients can live independently at home and within their wider communities.
This role offers a competitive salary of £48,000 plus the opportunity to earn a bonus. You'll enjoy the flexibility of working from home two days a week, with the remaining three days spent visiting clients in the community. - Conducting initial assessments in the community
- Providing clinical oversight to your team
- Collaborating with the wider multidisciplinary team (MDT)
- Attending meetings and making necessary referrals
- Managing a team of non-clinical case managers
The Community Nurse Case Manager role comes with a comprehensive package:
- Annual salary of £48,000
- Bonus opportunities
- Full-time, permanent position
- Monday to Friday, 9am-5pm with some flexibility
- Two days home-based, three days community visits
- Travel across Lincoln and surrounding areas
- RGN qualification with excellent clinical skills in complex care
- Experience with peg feeds, ventilators, and tracheostomy management
- A valid driving licence, as the role involves remote and community-based work
