Your Role:

Provide evidence-based care for acute and chronic conditions

Support health promotion, screening, and patient education

Collaborate with a multidisciplinary team to improve patient outcomes

What We Offer:

Competitive salary up to £45,000 p/annum pro rata (negotiable based on experience)

Part-Time hours up to 30 hours p/week

Requirements:

Registered Nurse with NMC registration

Experience in chronic disease management (Diabetes or Asthma)

Excellent communication and interpersonal skills
