As a Clinical Pharmacist, you'll play a pivotal role in enhancing patient outcomes. Your responsibilities will include:
- Conducting comprehensive medication reviews and assessments to ensure treatments are appropriate, effective, and safe.
- Collaborating closely with physicians and other healthcare providers to create personalised treatment plans.
- Providing enlightening drug information to healthcare teams and patients.
- Actively participating in medication reconciliation processes to minimise errors during care transitions.
- Contributing to the development and implementation of clinical guidelines and protocols.
- Engaging in continuous professional development to stay up-to-date with the latest pharmacy practice advancements.
- An annual salary between £36,576 and £44,759.
- A £3000 welcome bonus.
