Overview of Role:

As an RMN specialising in community care, you will play a crucial role in delivering patient-centered care:

• Conducting regular visits to patients in the community to assess and address their needs.
• Administering medication on schedule to manage patient conditions effectively.
• Engaging with patients to understand their conditions, monitor progress, and provide support as needed.
• Performing thorough risk assessments to ensure patient safety and well-being during treatment.

Requirements:

• Active registration with the NMC as a mental health nurse.
• Demonstrated experience in community settings.
• Strong communication skills and ability to thrive in a fast-paced environment.
• Capable of working autonomously and collaboratively within a team.
